What is Mirum Access Plus (MAP)?

MAP is a support program that helps patients and caregivers get medication and connect with helpful resources.

    MAP can help with:

  • Understanding insurance coverage and eligibility for financial support programs
  • Getting medication as quickly as possible
  • Connecting you with information and resources about Mirum medications

Mirum Access Plus offers MAP Mobile—a simple online tool that helps you navigate updates on your prescription, access helpful medication information, manage your refills, and provides educational resources.

When your doctor prescribes
CHOLBAM, there are
3 ways
you can enroll.

MAP Mobile

Your Navigator will give you access to MAP Mobile so you can digitally sign your consent form.

Download, sign, and send the MAP Consent Form

to MAP@mirumpharma.com, through mail, or by fax following the directions on the form.

Sign the consent form at your doctor’s office

When your doctor completes the enrollment form you may sign in person and MAP will contact you.

Getting Started

After your doctor prescribes CHOLBAM, a MAP Patient Navigator will call you and welcome you to the program. They'll collect information such as your insurance and provide you with support throughout the entire process.

Coordinating insurance coverage

MAP will review your insurance benefits and explore your options for financial support. If CHOLBAM is not approved by your insurance at first, we will have a team of experts and tools to help provide support on next steps.

Scheduling your delivery

Now it's time for CHOLBAM to ship to your doorstep. You'll receive a phone call from the MAP pharmacy to confirm your shipping information before sending your medication. After that, you'll receive ongoing support through all your refills of CHOLBAM so you never miss a dose.

Financial Assistance

Mirum Access Plus has been designed to help navigate insurance coverage, connect you to financial support that may be available, and keep you updated throughout the insurance coverage process.

MAP will help patients understand and navigate insurance and financial support resources related to their medication.

Mirum Access Plus Savings Program

Eligible patients with commercial or private insurance may pay as little as $0 out of pocket per fill for CHOLBAM.*

Mirum Patient Assistance Program (PAP)

Our Patient Assistance Program provides support for those who may require and qualify for financial support.* Our programs are matched to the patient's needs and eligibility criteria.

Our dedicated MAP team can help determine eligibility for available support options and guide patients through the enrollment process.

With MAP, you have ongoing support at every stage of your CHOLBAM journey.

Our goal is to be a reliable partner throughout your CHOLBAM journey, focusing on what matters most to you.

You will have a dedicated Patient Navigator who stays with you from the start, ready to answer questions, listen to your concerns, and connect you with helpful resources. Your doctor will always guide your treatment decisions, and we are here to support you every step of the way.

Mirum Access Plus is a comprehensive patient support program that is activated by enrolling and consenting for support. MAP offers reliable resources and access to Patient Navigators throughout your journey with our medications.

MAP can help you with understanding your insurance, supporting the insurance approval process, answering questions about your medication, and connecting eligible patients with financial assistance options to ensure you are well supported throughout the journey.

There are lots of options to enroll. Patients can call 1-855-MRM-4YOU (1-855-676-4968) to sign up for MAP Mobile, complete and submit your form via email, mail, or at your doctor's office.

MAP Mobile is available on your phone or desktop, where you can see updates about your prescription, get refill reminders, submit refill requests, and access all things related to your treatment with CHOLBAM.

CHOLBAM will be delivered right to your doorstep from the MAP pharmacy. MAP will help you with getting insurance coverage for CHOLBAM and help connect you with any potential financial assistance you may be eligible for.
